Lung Nodule Evaluation: Causes, Diagnosis, Tests and Treatment

A lung nodule is a small, rounded or irregular area of abnormal tissue in the lung that is usually detected on a chest X-ray or CT scan. Lung nodules are common, and most are not cancerous. They can develop because of previous infections, inflammation, scar tissue, benign tumors or other conditions. However, some nodules can represent an early-stage lung cancer, which is why appropriate evaluation and follow-up are important.

The evaluation of a lung nodule is not based on its size alone. Doctors consider its appearance on CT, growth over time, location, the patient's age and risk factors, and whether there are other abnormalities in the lungs or chest.

What Is a Lung Nodule?

A pulmonary or lung nodule is generally described as a rounded opacity measuring up to approximately 3 cm in diameter. Lesions larger than 3 cm are usually referred to as lung nodule and have a different level of concern.

Nodules may be:

  • Solid nodules

  • Part-solid nodules

  • Ground-glass nodules

The type and appearance of a nodule can provide important information about its possible cause and help determine the appropriate next step.

A lung nodule may be found incidentally during a CT scan performed for another reason, such as evaluation of chest pain, cough or a respiratory infection.

What Causes Lung Nodules?

There are many possible causes of pulmonary nodules. Common benign causes include previous infections and inflammatory conditions.

Potential causes include:

  • Healed bacterial or fungal infections

  • Tuberculosis and previous granulomatous infections

  • Inflammatory lung diseases

  • Rheumatoid or autoimmune conditions

  • Benign lung tumors

  • Scarring from previous lung injury

  • Primary lung cancer

  • Less commonly, cancer that has spread from another part of the body

The presence of a lung nodule does not automatically mean that a person has lung cancer. Proper evaluation is necessary to estimate the likelihood of malignancy.

Risk Factors for a Cancerous Lung Nodule

Several factors can increase the likelihood that a pulmonary nodule is malignant. These include increasing age, a history of cigarette smoking, previous cancer, significant exposure to certain occupational substances and certain characteristics seen on CT imaging.

A person's overall clinical history is therefore an important part of lung-nodule evaluation.

Doctors may classify a nodule as having a low, intermediate or high probability of malignancy.lung nodule This risk assessment helps determine whether surveillance, additional imaging, biopsy or treatment is appropriate.

CT Scan in Lung Nodule Evaluation

A thin-section chest CT is one of the most important tests for evaluating a pulmonary nodule. CT provides much more detail than a standard chest X-ray.

Doctors assess several characteristics, including:

  • Nodule size

  • Shape and margins

  • Density

  • Location

  • Presence of calcification

  • Relationship to nearby structures

  • Whether the nodule is solid or subsolid

  • Whether there are additional nodules

  • Changes compared with previous scans

Features such as irregular or spiculated margins can increase suspicion, although imaging findings alone cannot always establish a diagnosis.

Importance of Previous CT Scans

One of the most valuable pieces of information in lung-nodule evaluation is whether the nodule has changed over time.

Comparing the current CT scan with older imaging can show whether a nodule is stable, growing or newly developed. Some nodules remain unchanged for long periods and may require only appropriate surveillance.

Growth, particularly when demonstrated on serial CT examinations, can increase concern and may lead to additional investigation.

Patients should therefore provide their doctor with previous chest X-rays or CT scans whenever possible.

PET-CT for Lung Nodules

In selected patients, a PET-CT scan can provide additional information. PET imaging evaluates metabolic activity within a lesion and can help assess whether a nodule is suspicious.

However, PET-CT is not perfect. Some cancers may have relatively low metabolic activity, while infections and inflammatory conditions can also show increased uptake.

Therefore, PET-CT findings must be interpreted together with CT characteristics, clinical risk factors and other investigations.

When Is a Lung Biopsy Needed?

Not every lung nodule requires immediate biopsy. For small nodules with a low estimated risk of cancer, doctors may recommend CT surveillance at appropriate intervals.

A biopsy may be considered when a nodule is suspicious, enlarging, sufficiently large, or when obtaining a tissue diagnosis would change management.

Depending on the nodule's location, biopsy options can include:

  • Bronchoscopy

  • Endobronchial ultrasound-guided procedures

  • CT-guided percutaneous biopsy

  • Surgical biopsy

The safest and most appropriate technique depends on the size, location and characteristics of the lesion, as well as the patient's overall health.

Role of Bronchoscopy

Bronchoscopy allows a pulmonologist or interventional pulmonologist to examine the airways and obtain tissue samples from appropriate lesions.

Advanced navigation and image-guided bronchoscopic techniques may help doctors reach certain peripheral lung nodules that are difficult to access with conventional bronchoscopy.

When enlarged lymph nodes are also present, EBUS-TBNA (endobronchial ultrasound-guided transbronchial needle aspiration) may allow sampling of lymph nodes in the chest. This can be particularly useful when evaluating a suspected lung cancer.

Lung Nodule Surveillance

For nodules that do not immediately require biopsy or treatment, doctors may recommend follow-up CT scans. lung nodule The timing depends on factors such as nodule size, appearance, type and estimated cancer risk.

Surveillance is designed to identify meaningful growth while avoiding unnecessary invasive procedures in patients whose nodules are likely benign.

Patients should follow the recommended imaging schedule rather than assuming that a stable or small nodule does not require follow-up.

Treatment of a Suspicious or Cancerous Nodule

If testing confirms that a lung nodule is cancerous, treatment depends on the type and stage of cancer.

For an early-stage lung cancer confined to the lung, treatment may include surgical removal of the tumor. Selected patients who cannot undergo surgery may be considered for stereotactic body radiation therapy.

If cancer has spread to lymph nodes or distant organs, treatment may involve chemotherapy, targeted therapy, immunotherapy, radiation therapy or combinations of these treatments depending on the cancer's pathology and molecular characteristics.

When Should You See a Specialist?

A person should discuss a newly discovered lung nodule with a qualified doctor, particularly when the nodule is growing, has suspicious CT characteristics or occurs in someone with significant risk factors for lung cancer.
